Player Physicals
ALL players who plan on participating in football during the 2023 season need to have an Up-to-Date physical card on record in the Holmen High School Office. Any player who has had a physical ON or AFTER April 1, 2022 is good to go for the 2023 season. If a player had a physical PRIOR to April 1, 2022 they will need to get a physical to be eligible to participate during practices. Please have all physicals completed and turned in prior to July 17, 2023 so we can avoid last second problems.
You can use the button below to download a copy of the physical form that players will need to have completed. ONLY page 4 needs to be returned to Matthew Sullivan in the Holmen High School Office. Please make sure everything on page 4 is filled out before you turn in the form.
